Player Development Classes Open to 6-18yo
2020 Weekly schedule:
11 years old and younger (Choose 1 or 2 days per week)
Monday: 7:00 to 8:00 PM
Wednesday: 6:00 to 7:00 PM
12 years old and older (Choose 1-3 days per week)
Monday: 8:00 to 9:00 PM
Tuesday: 8:00 to 9:00 PM
Wednesday: 8:00 to 9:00 PM

I/We the undersigned hereby certify that I/We am/are the parent(s) or legal guardian(s) of the student.
Consent for medical treatment
I/We hereby give permission for the staff of the River Plate Indiana Camp to seek appropriate medical attention for the student and for the medical attention to be given and for the student to receive medical attention in the event of accident, injury or illness. I/We will be responsible for any and all costs of medical attention and treatment, except for that covered by the camp’s excess medical coverage policy.
I/We hereby acknowledge that our child is physically fit and mentally capable of participating in soccer camp activities. I/we hereby acknowledge that playing or practicing soccer is a potentially hazardous activity. I/we agree that I/we am/are voluntarily entering and assuming all risks associated with participating in those activities, including but not limited to falls, contacts with other participants, the effects of weather including heat and or humidity, and the conditions of the training grounds and facilities, all risks being understood and appreciated by you. I/We, the undersigned for ourselves, our heirs, executors and administrators hereby waive, release, and forever discharge Juncos Entertainment Group LLC staff, officers, agents, employees, representatives and successors and assign of and from all rights and claims for damages, injury or loss to person or property which may be sustained or occur during participation in Camp activities or while at camp, whether or not damages, injury or loss is due to negligence. I also hereby acknowledge that no refunds will be given due to any type of injury occurring during the camp.
Use material for promotional purposes
I hereby grant the Juncos Entertainment Group LLC permission to use my likeness in a photograph, video, or other digital media (“photo”) of this event in any and all of its publications, including web-based publications and social media, without payment or other consideration.
I understand and agree that all photos will become the property of the Juncos Entainment Group and will not be returned.
I hereby irrevocably authorize the Juncos Entertainment Group LLC to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.
I hereby hold harmless, release, and forever discharge the Juncos Entertainment Group LLC from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
In the event of inclement weather once camp begins, all campers will be asked to leave the field to the designated shelter area. If camp is cancelled once started, you will be asked to return to the field and pick up your player. We do not have indoor facilities to house players for the rest of the day.
Cancellation & Refund Policy
All fees are 75% refunded if the request is made in writing via certified mail or e-mail sent to email@example.com (with confirmation of receipt) within 45 days of the start of the camp. There will be no refunds after the camp has started. No refunds are made when a request is made with less than 45 days remaining to the start of each camp regardless of when payment is made. Camps are subject to change or cancellation without notice at any time for any reason, including if the number of participants registered is insufficient to host the camp. Upon cancellation of camp by our staff, full refunds are issued within 30 days of cancellation or participants can choose to attend any of the other camps based on availability. If the Camp is cancelled, our liability is limited to a full refund of the price and we will not be liable for any other costs, damages, or refunds of any kind for any loss, delay, inconvenience, disappointment, travel or other expense whatsoever in such circumstances. No refunds will be given if a Camp in progress has to be interrupted or cancelled for reasons beyond our control.
Players will have the opportunity to purchase Registration Protection during the checkout process at a non-refundable fee of $50. This protection allows players to receive a FULL refund up until 30 days prior to camp start. No refunds are given within this 30 day period.
